Tips to Avoid Re-Hospitalization

According to the Centers for Medicare and Medicaid (CMS), “More than 15% of Medicare patients experience an adverse event during their hospital stay, and a third require readmission within a month of release.”

If your loved one was recently discharged from the hospital, please take this news to heart. Your loved one may need short-term service after leaving the hospital.
Learn more about our services for information about short-term assistance during this critical month following discharge from a hospital.
Creating a “recovery environment” at home or in a rehabilitation center is possible. Our short-term services may make a significant difference in the full recovery of your loved one.
According to CMS, the first medical appointment after discharge from the hospital is crucial. We can help to ensure that this medical visit is possible with our short-term service, and we may provide our incidental transportation if a family member can not be there.
